Six day advanced accident investigation and causal analysis - three two-day sessions

This set of two-day courses are designed to be run either as three separate sessions, or as individual CPD style updates.  The courses are designed for those who have undergone some basic training in accident investigation and now wish to take their skill set to a higher level.  The final two-day session is designed to provide delegates with the skills to develop a formal data trending process enabling quarterly reports to the executive board outlining identified holistic weaknesses within the organisation within safety (Business) culture and process.  The 6 days are set out as follows:

Session one 2 days:
Putting loss events into context; initial collecting and coding of data

Action at the scene, recording the scene, data types and understanding data
Identifying a set of organisational failure areas that can be used to analyse organisational performance - Axial coding
Examining how data can be used to identify the organisational failure areas and weaknesses in culture that led to the loss environment
Examining a recognised academic system of coding qualitative data (both organisational and cultural)
Examining a recognised academic system of trending data (both organisational and cultural) Thematic coding of qualitative data
Recording data in the form of (Criminal justice Act) CJA statements in a way that is academically acceptable and of use to the legal profession in the role of defence
(UK Only) English legal statement taking rules, the effects of PACE and CPA

Session two 2 days:
Advanced interviewing

Understanding the interview process
Witnesses/ research subjects, rather than suspects
Issues surrounding witnesses
Theme development and category sets
Interview preparation
Types of questions
Interviewer bias and how to overcome it
Control of false hypothesis within interviews
Room setting
Effective listening
The importance of effective listening during first recalls
Interviewing exercise
Oppression and interviewing
Stress management during the interviewing process

Session three 2 days:
Facilitated discussion and executive governance of the workplace environment

Upstream categorisation of decision making issues
The role of senior decision makers
Developing a cultural positionality
Using trended critical themes to stimulate positive evolution of the culture
Identifying issues surrounding group-think
Examining decision making factors
Providing a format for a facilitated discussion paper
Key questions associated with developing safety culture and their links to governance

The cost of each session is £5000 plus vat and expenses for up to a maximum of 12 delegates.

6 HOUR CPD COURSE FOR DIRECTORS ON MODERN DAY ACCIDENT INVESTIGATION

This new version of the course takes delegates on a journey examining some new ideas and concepts as well as questioning some of the older established rhetoric.

​

Delegates will be provided with an insight into how investigation results can provide a basis on which to develop organisational safety culture.  Additionally they will be provided with an insight into how investigative data can be used to constantly monitor core business standards and ethical positionality.

FOUR DAY LEAD INVESTIGATORS COURSE - INTERNATIONAL

This course can be run over 4 or 5 days depending upon the needs of the delegates.  It is aimed at those who have responsibility for gathering and analysing data.  The course examines loss events as being 'symptoms' of weakness in the management of certain core functions in the development of work processes and  transmitted values and attitudes from the senior management team.

​

The process uses recognised academic processes for gathering and analysis of qualitative data.  Delegates are taught how to address scenes, interview witnesses, gather exhibits, analyse their information and then examine results in the light of similar data from other events

FOUR DAY LEAD INVESTIGATORS COURSE UK BASED

The primary aim of the course is to provide delegates with a system with which data can be collected, presented and analysed in accordance with both legal and academic standards.  Delegates will be encouraged to collect data and then examine it using a set of academicaly recognised organisational failure areas and a corresponding set of qualitative behavioral indicators.

​

Both this course and the international course are very intensive and require attendance each day if delegates are not to left behind.  This course is pitched at about 'taught masters level', not because we are trying to make it more difficult than it should be, but simply because academically this is where the discipline sits.  If delegates feel that they may have difficultly with the educational levels its suggested that they confidentially speak with the tutors who are experienced in assisting adults to get the most from the material.

This course is very similar to the international course, however it is taught through the medium of the English law that so heavily controls the investigative process.  Delegates examine the effects of PACE, GDPR the rules surrounding criminal justice act statements, issues surrounding working with the authorities and openness within investigations and issues surrounding data handling.

TWO DAY ACCIDENT INVESTIGATION COURSE - SENIOR MANAGERS 

This two day syllabus is designed for those managers who may have investigations occurring in their workplace, but who will never actually be involved in one themselves.  The idea to ensure that any investigation is not impeded by out of date processes.

​

It is very easy to say that these individuals do not need any input, however it can be very disruptive and disconcerting to have an investigation taking place and all the confidentiality surrounding it without understanding the needs and the process.

​

After some 19 years of providing this input we can say with confidence that it takes two full days to provide the necessary information.

TWO DAY ACCIDENT INVESTIGATION COURSE - MANAGERS AND SUPERVISORS

This two-day session is designed for managers and supervisors who have a responsibility to investigate accidents within their areas of responsibility and have the support of mentors who have completed the four day lead investigation course.  These investigators are trained to be able to gather data that identifies what took place when the events occurred. This course covers both human failure and human belief issues, together with an examination of organisational failures.

Delegates are instructed in interview and statement taking skills.  Plus they are provided with a set of basic, but generic actions to be carried out at the scene.  There is discussion in relation to the role of the authorities and issues surrounding the provision of assistance and data to these investigators.

​

It is strongly recommended that investigators at his level have the necessary support within the organisations to enable data to be collected and analysed in a meaningful way.

BASIC RESPONSE AT THE SCENE

This is a four hour session designed to provide front-end operatives with the basic knowledge of what to do when an event takes place.  All too often there is a drive to continue with work and ‘clear up’.  As a result, good data is lost to subsequent investigations due to the actions of well-meaning staff.

This session is run by in-house trainers. It is a very basic course, which leads to a small test and a certificate.  

The course covers a basic set of agreed actions that were developed by an accident investigation working party that included Police, HSE, IOSH and Kinaston Associates personnel.  It is assumed that any qualified HSE professional should be able to facilitate the session.

Kinaston Associates will train in-house staff in presenting the course and facilitating the certificate issuing process.
